    An endoscopic anatomic study of the ventral craniocervical junction with the assistance of neuronavigation

    Objective The aim of this study was to evaluate the feasibility of endoscope assisted far-lateral approach with neuronavigational guidance and to observe the anatomic structures of ventral craniocervical junction identified with the endoscope. Methods Six fresh cadaveric specimens were prepared for a far lateral approach to allow for microscopic, 0 degree endoscopic and 30 degrees endoscopic view. By drilling off the jugular tubercle and one-third of the occipital condyle with neuronavigation guidance, the increase in exposure and neurovascular structures visualization with the microscope were then evaluated. The exposure of the petroclivus area provided by the endoscope and by the operating microscope was measured and compared. Results Clear close-up views of the all neurovascular structures in deep ventral craniocervical junction and structures normally obstructed by the jugular tubercle and occipital condyle were obtained using the endoscope through three corridors enclosed with cranial nerves with neuronavigational guidance, as similar with far lateral transcondylar approach. Conclusion With the aid of the endoscope and neuronavigation guidance, the far lateral approach is a feasible procedure that allows the identification of all neurovascular structures in ventral craniocervical junction without drilling the jugular tubercle and occipital condyle and may be used to remove lesions involving in this area.

    Objective Study on anatomy of endoscopic transoral approach to jugular foramen, in order to the application of this approach to remove the communicating tumor in jugular foramen region.  Methods With the aid of endoscope,15 cadaveric head specimens were used to simulate transoral approach to jugular foramen. The extracranial and intracranial structures of jugular foramen were observed, the anatomic landmarkers were located and the relevant data measured. Results Transoral approach could reveal extracranial region of jugular foramen well, especially the anterior medial region. The best exposure to the midline region of the brain stem was obtained after removing the bone from the external opening of the hypoglossal canal to the midline of the clivus. The occipital condyle, supracondylar groove and carotid ridge were important anatomical landmarks in this approach. The distance was (14.51±2.30) mm from the anterior edge of the occipital condyle to inferior edge of the external opening of the hypoglossal canal. The distance was (24.11±2.19) mm from anterior edge of the occipital condyle to carotid ridge. The distance was (21.26±2.26) mm from anterior edge of the occipital condyle to the upper edge of the jugular tubercle. Conclusion  This approach is helpful in dealing with the principal part of tumors in the medial region of the jugular foramen.

    Morphological characteristics of abnormal obturator arteries and its clinical significance

    Objective To investigate the shape, distribution and variation of abnormal obturator artery and vein in the pelvic wall, and to measure the length of the segment of obturator artery in the pelvic cavity, in order to provide a reliable anatomical reference of etiology to the strange bleeding around the area.  Methods There were 37 specimens of adult corpses fixed by the formaldehyde solution the pelvic dissection. According to the obturator artery classification method, these specimens were investigated and classified. The length of the segment of the obturator artery in the pelvic cavity, and the external diameter of it at the origin and at the obturator foramen, and the distance between the obturator foramen and the midpoint of the line connecting anterosuperior iliac spine and the pubic symphysis were explored. Results In these 37 specimens, the occurrence rate of abnormal obturator arteries was 18.92%. A common trunk of abnormal obturator arteries and inferior epigastric artery, length of which was(1.57±0.12)cm, originated from the external iliac artery. The length of the abnormal obturator artery was(4.05±0.49)cm, and the external diameter of it at its origin and at the obturation foramen was(2.52±0.87)mm and(2.14±0.72)mm, respectively.  The average distance between the obturator foramen and the midpoint of the line connecting anterosuperior iliac spine and the pubic symphysis was(3.63±1.29)cm. Conclusion The obturator artery is relatively thicker and longer in the pelvis, and has a high variation rate. Abnormal obturator arteries originate from inferior epigastric artery, and then bypasses or crosses, and vertically accesses into obturator foramen.  It is due to the special characteristics and distribution of obturator arteries, the surgeon should pay attention to its existence in inguinal surgery and pelvic fracture fixation operation, or even in oncotherapy and clearance of lymph nodes. In addition, it is better for learning about the obturator area in advance by proper usage of angiography and to apply the lateral rectus approach in the abdominal surgery. Besides, minimally invasive surgery might be another choice.

     Objective    The purpose of the study is to describe the precise anatomical  characteristics of the Bassett's ligament, and to explore the physiological function and the clinical significance.    Methods    The study was performed on 10 ankle joints of frozen-fresh cadaveric ankles. The precise location of origin,insertion points, and topogeography were carefully observed,and the precise anatomical parameters(their length, width and thickness)were also recorded.    Result    The Bassett's ligament was present in 8 of 10 ankles, and ran obliquely from anterior tibial tubercles to anterior peroneal tubercles. The shape was quardrangular. Its orientation was parallel and distal to the anterior tibiofibular ligament, its length ranged from 17.86 to 18.56 mm, its width from 3.22 to 3.89 mm,and its thickness from 0.67 to 1.47 mm.  Conclusions   The Bassett's ligament reflects a normal anatomical finding, and the cause of tibiotalar impingement syndrome can be explored by investigating the simulated mechanism of injury of the ligament.

    Objective To provide evidence for the morphological diagnosis on the oppression and contact of the cisternal segment of facial nerve with its surrounding vessels. Methods 140 normal and 70 patients with unilateral hemifacial spasm were selected by MRI 3D-FIESTA and 3D-TOF sequence scanning. The following parameters were observed in the MPR reconstruction image, including the vascular diameter, distance to brainstem vascular self angle and included angle at the intersection of the cisternal segment of facial nerve with its surrounding vessels, and its relationship with the morbidity of hemifacial spasm. Results The vascular diameter, distance to brainstem, vascular self angle and included angle at the intersection of the cisternal segment of facial nerve with its surrounding vessels were (0.48±0.03)mm, (0.49±0.04)mm, (0.71±0.06) mm and (8.69±0.62) mm, (8.66±0.75)mm, (5.93±0.47) mm and (74.39±2.33) °, (72.66±2.54) °, (48.57±3.28) ° and (85.60±3.07) °, (86.75±2.73) °, (88.41±3.76) ° on the normal group, uninjured group of hemifacial spasm, ipsilateral group of hemifacial spasm, respectively. There were significant difference between the ipsilateral group of hemifacial spasm to the normal group, uninjured group of hemifacial spasm at the vascular diameter, to brainstem distance and vascular self angle at the intersection of the cisternal segment of facial nerve with its surrounding vessels (P<0.05), there was no significant difference at the included angle(P>0.05). The morbidity decreased with the increase of intersection to brainstem distance, increased with the enlargement of vascular diameter and the reduction of vascular self angle. There was no significant change between the included angle to morbidity. Conclusions The vascular diameter, distance to brainstem and vascular self angle at the intersection of the cisternal segment of facial nerve with its surrounding vessels are important diagnostic markers whether the oppression or contact exists. The lesser of the vascular self angle, the larger of the vascular diameter and the shorter of the distance between the intersection and brainstem, the more prone the patient is to hemifacial spasm lesions.

    Objective To explore the application value of high-frequency color Doppler ultrasound for marking the femoral anterior-medial side perforator artery before operation and monitoring of postoperative haemodynamics.  Methods  56 patients with skin and soft tissue defect underwent perforator flap reconstruction. The high frequency color Doppler ultrasound was used to examine femoral anterior-medial side perforator artery before operation. The diameter, blood flow and haemodynamics indexes (Vmax,Vmin,PI and RI)of the perforator artery were detected before the operation and at 1th,3rd, 5th, 7th, 14th and 30th day after the operation. Results There were 102 good femoral perforator arteries at the anterior-medial side of 56 thighs. The diameter, Vmax, Vmin and flow of the perforator artery gradually increased at the 1st, 3rd, 5th and 7th day after the operation, and decreased at the 14th and 30th day. The diameter, Vmax, Vmin and flow of the perforator artery after operation were higher than those before operation, and there were significantly statistic differences between them.The PI and RI gradually decreased at the 1st, 3rd, 5th, 7th and 14th day after operation and then increased at the 30th day. The PI and RI of the perforator artery after operation were lower than those before operation, and there were significantly statistic differences between them.  Conclusion  High frequency color Doppler ultrasound could visually display body surface localization and haemodynamics indexes of femoral anterior-medial side perforator artery, which could better provide guidance for design before operation for flap, and detect the vascular crisis early and evaluate the prognosis.

    Objective To explore the segmentation method of fetal hepatic vascular three-dimensional model and its significance. Methods One case of fresh induced labor normal fetal liver(38 week)was infused with color separation and density contrast metal contrast agent and then casted, red epoxy resin filler perfusion of abdominal aorta (contrast agent for lead oxide), blue epoxy resin filler perfusion of inferior vena cava, ductus venosus, left hepatic vein and lateral branch of right hepatic vein (contrast agent for titanium dioxide), purple epoxy resin filler perfusion of intermediate hepatic vein(contrast agent for zinc oxide), green epoxy resin filler perfusion of interior branch of right hepatic vein(contrast agent for lead oxide).the 128-slice spiral CT thin layer scanning and Mimics software were used to reconstruct three dimensional model of fetal liver. Results The three dimensional model of fetal hepatic vessels was constructed, which were clearly and solid. When the Mimics threshold spacing was progressively smaller, it can effectively segment the intermediate hepatic vein, the hepatic vein system and the hepatic proper artery, which can clearly show the course and distribution of the multiple vessels in fetal liver. Conclusion  It is simple and feasible to apply different density contrast agents to perfusion of multiple vessels in fetal liver, which can provide ideal technical support for the study of the development and transplantation of fetal hepatic vessels.

    Anatomy of the aberrant cervical internal carotid artery under CT angiography and MRI

    Objective  To describe imaging anatomy features of the aberrant cervical segment of internal carotid artery through CTA and MRI skill.  Methods The radiological data of 389 cases between Jan. 2012 to Dec. 2016 were collected and analyzed, who underwent head or head-neck CTA due to cerebrovascular disease or health examination. 15 cases were recognized with the aberrant cervical segment of internal carotid artery, 1 case of aberrant internal carotid artery was identified under 1.5T MRI. Abnormal location and the line exception form of cervical segment of internal carotid artery were observed and described.  Results Three forms of abnormal location of cervical segment of internal carotid artery coulb be find under CTA and  MRI: 1. submucosal position at the oropharynx; 2. submucosal position at the nasopharynx; 3. pass through the paprapharynx spcae and far away from the oropharynx or nasopharynx cavity. The line exception form included >or<-shaped, C-shaped, cross-U-shaped, S-shaped, Z- or N-shaped, coiling-shaped, or 3-shaped.   Conclusions CTA examination can be used to observe the local axial abnormal contorts of the artery,  moreover, by VR post-processing technology, the overall artery contorts can be demonstrated, which is a reliable clue for clinical pharyngeal surgery and cerebrovascular disease diagnosis.

    Research on the reactivity of chitosan/ SiO2/hydroxyapatite hybrids in vivo

    Objective To analyze the reactivity of chitosan/ SiO2 (CS-SiO2) and chitosan/SiO2/hydroxyapatite (CS-SiO2-HA) hybrid materials in vivo.  Methods CS-SiO2 or CS-SiO2-HA hybrid materials were implanted respectively into the gastrocnemius muscle of C57BL/6 mice and the control group was set up (gastrocnemius blunt dissection after direct suture). Frozen sections of materials containing the gastrocnemius were removed 14, 28, 42, 56 d after implantation, followed HE staining and immunofluorescence to observe local inflammation, muscle fibrous necrosis and regeneration induced by the implanted materials.  Results Histological observation showed that in the early stage of intramuscular implantation (14d), CS-SiO2 and CS-SiO2-HA hybrids both induced inflammatory cell infiltration, and the latter was more significant. Number of infiltrated cells began to decrease after 28 d and intramuscular inflammation in the group of CS-SiO2 almost disappeared while there was a small amount of infiltration in CS-SiO2-HA at 56 d. Immunofluorescence further confirmed that CS-SiO2-HA hybrids led to more severe necrosis of muscle fibers, triggered more extensive and prolonged inflammatory infiltration of mononuclear macrophages compared to CS-SiO2. Intramuscular inflammation basically disappeared and the repair of muscle fibers was completed two months after transplantation of CS-SiO2 and CS-SiO2-HA hybrids. Conclusions CS-SiO2-HA and CS-SiO2 hybrid materials can both induce a short-term and local inflammatory reactions while CS-SiO2 has a better compatibility in vivo than that of CS-SiO2-HA hybrids.

    The effects of edaravone combined with breviscapine treatment on MCP-1 expression in the cerebral cortex after focal cerebral ischemia in rats

    Objective This research investigated the expression of MCP-1 in cerebral cortex of rat following middle cerebral artery occlusion (MCAO) and edaravone combination with breviscapine as a treatment of MCP-1 expression. Methods Animal models of MCAO were established, and RT-PCR,Western blot and Immunohistochemistry were used to detect the expression of MCP-1 in cerebral cortex of rats after MCAO and drug intervention.  Results  The expression of MCP-1 mRNA were significantly enhanced after MCAO, and became most severe at 12 h, and the difference was significant (P<0.05). The expression of MCP-1 mRNA was markedly decreased compared with the saline group after drug treatment (P<0.05); The combination of edaravone plus breviscapine was more effective than edaravone or breviscapine  treatment alone (P<0.05). The expression of MCP-1 protein was significantly increased in 1d, 3d, 1 week following MCAO and stayed at a high level (P<0.05), and than obviously decreased compared with the saline group after drug treatment (P<0.05); the combination of edaravone plus breviscapine was more effective than edaravone or breviscapine alone (P<0.05) . Immunofluorescence staining showed that activated microglial cells was co-labeled with MCP-1 positive cells, and the number of MCP-1 positive cells was significantly increased after MCAO peaking at 1week. By the mean time, microglial cells did not exhibit MCP-1 positive cells in saline group. Conclusion Edaravone combination with breviscapine as a treatment could decrease the MCP-1 expression after cerebral ischemia, and the combination of edaravone plus breviscapine is more effective than edaravone or breviscapine alone.

    The effect of different concentrations of cytochalasin D on the differentiation ability of human adipose-derived stem cells

    Objective To observe the effect of different concentrations of cytochalasin D (Cyto D) on the differentiation ability of human adipogenic stem cells to osteogenesis and adipogenesis. Methods Different concentrations of Cyto D that 0.05 μg/ml, 0.1μg/ml and 0.2μg/ml were added to growth medium, osteogenic differentiation medium and adipogenic differentiation medium, respectively, to disturb the elongation of cytoskeletal actin in different environments. Then the changes of cell differentiation ability were detected by oil red O staining, ALP staining and CCK8, in the treatment of 1d, 4d and 7d. Results Cyto D can both induce the formation of lipid droplets and change the basal state of osteogenesis in hASC. The combination of Cyto D and adipogenic differentiation medium can greatly promote the lipids formation. Low concentrations Cyto D increased the number of lipid droplets, and high concentrations Cyto D promoted the maturation of fat droplets. In combination with osteogenic differentiation medium, Cyto D was inversely proportional to the concentration and osteogenic effect. Conclusion Cyto D can enhance the effect of differentiation medium. Low concentration Cyto D mainly promotes osteogenesis and high concentration of Cyto D mainly promotes adipogenesis. Our exploration on the effect of myosin on the differentiation of adipogenic stem cells induced by Cyto D can provide important biological information for studying the differentiation mechanism of adipogenic stem cells.

    The research of specified recognition of lectins with carbohydrate modified on surface markers of mouse embryonic stem cells

    Objective Identification of lectins which can specified recognize the carbohydrate modified surface markers on mouse embryonic stem cells. Methods In the present study, a panel of 3 lectins and carbohydrate antibodies was used to characterize the carbohydrate surface markers of mouse Embryonic Stem (ES) Cells by Flow cytometry assay and Immunocytochemistry. Results  Enrichment of mouse ES cells yielded approximately 99% of SSEA-1-positive mouse ES cells. A uniform and high percentage of binding was observed for DSL, having similar percentage of binding to SSEA-1 (99%). Partial binding of WFL was observed in mouse ES cells which was also reflected by the respective immunocytochemistry images. A very low percentage of binding was observed for UEAI. Conclusion DSL can be used to determine pluripotency which shows similar binding to SSEA-1, a well-established pluripotent marker and UEAI is a negative marker. Taken together, the data has provided information on the cell surface carbohydrate profile of mouse ES cells.

    The expression and significance of active oxygen modulation factor 1 in human intervertebral disc degeneration

    Objective To explore the expression and their relationship of ROMO1 and ROS in the degenerative intervertebral disc. Methods Immunohistochemical techniques and fluorescent probes DCFH-DA were applied to detect the expression of ROMO1 and ROS levels in the different degenerated nucleus pulposus. Results The mean option density of ROMO1 were 0.192±0.089, 0.328±0.048, 0.399±0.053, and 0.468±0.098, respectively. The results of ROS detection were 132.961±15.149, 191.889±17.880, 218.056±12.845, and 243.501±30.279, separately,in the groups of A, B, C, and D.  It showed the expression of ROMO1 and the content of ROS increased gradually with the aggravation of intervertebral disc degeneration, and there was a positive correlation between ROMO1 expression level and the content of ROS in intervertebral disc (r=0.732, P<0.05). There was also a positive correlation between ROMO1 expression level and the intervertebral disc degeneration degree (r=0.603, P<0.05). Conclusion ROMO1 and ROS are highly expressed in the nucleus pulposus tissue of the degenerated intervertebral disc, which has a positive correlation;  and the expression level of ROMO1 is positively correlated with the degree of degeneration of intervertebral disc, which might be related to aggravated oxidative stress injury by increasing the content of ROS in the nucleus pulposus.

    Objective To invatigate the effect of Tongxinluo on transforming growth factor beta 1(TGF-β1)and connective tissue growth factor (CTGF) expression in diabetic rat kidney, and effect of tongxinluo on diabetic rats kidney protection. Methods The 40 healthy male SD rats were randomly divided into 2 groups, with 12 rats in the normal control group (CON group), and 28 rats in the model group. The model group was used to manufacture diabetes mellitus with a disposable tail vein injection of Streptozotocin (STZ) 55 mg/kg, and the blood glucose was greater than 16.7 mmol/L after 72 hours. 24 rats with successful modeling were randomly divided into diabetic group (DM group), and group (TXL) tongxinluo group, 12 rats in each group. 8 weeks later full automatic biochemical analyzer test was used for biochemical indexes and kidney tissue ultrastructure was observed using electron microscopice. Immunohistochemical method was to detect the expression of TGF-β1, and CTGF on protein level, Western blot method was used for determination of TGF-β1, and CTGF protein levels in kidney tissue. Results Compared with the diabetic group, the thickening of the renal basement membrane, the accumulation of mesangial matrix and the fusion of the foot were all reduced, and the expression of TGF-β1 and CTGF in renal tissues decreased.  Conclusion  Tongxinluo can reduce the expression of TGF-β1 and CTGF in kidney tissues of diabetic rats, and has protective effect on kidney of diabetic rats.

    Objective To compare the difference of loose connective tissues from the subcutaneous and mesenteric membrane of Wistar rats. To compare the difference in staining of loose connective tissue from different sites and with different staining methods.  Methods Wistar rats were continuously injected intraperitoneally with 2.5 ml of trypan blue saline solution (10 g/L) once a day for three days. Loose connective tissues were taken from the subcutaneous and mesentery of rats, respectively. Two sites were treated with HE staining and aldehyde fuchsin-bright green-orange yellow G staining. Results In subcutaneous extraction with HE staining, a large number of fibroblasts could be seen. Mast cells were obvious and macrophages were visible. Elastic fibers and collagen fibers were visible, but not obvious. In Aldehyde fuchsin-bright green-orange G staining, elastic fibers and collagen fibers were stained into blue-violet and collagen Orange, respectively. But cells were not colored. In mesenchymal material with HE staining, fibroblasts, mast cells and macrophages could be found. Elastic fibers were stained into blue and purple, while collagen fibers were stained into light red. Aldehyde fuchsin-bright green-orange G staining stains elastic fibers and collagen fibers into red and Bright green, respectively. The color of mast cells was red, and the nucleus was brown. Macrophages were clearly visible with irregular shape. Thick blue-violet particles could be seen in the cytoplasm, and the nucleuses are round and brown. The cytoplasm of fibroblast cytoplasm could be colored, while the nucleuses were stained into brown. Conclusion Aldehyde fuchsin-bright green-orange G staining for loose connective tissue from inferior mesenteric is a simple and suitable method for stretch preparation of loose connective tissue in rats.

    Objective To analyze the influence of different end-range cervical rotation on the vertebral artery lumen with CTA.    Methods    To trim 8 fresh cervical adult specimens with upper cervical spine(C0~4) is used to build the vitro model loaded 3 kg of vertebral artery. Contrast agent is injected into vertebral artery and the vertebral artery cross-sectional area in the section(C1,C1/2, upper part of C2 vertebral body 1/3, lower part of C2 vertebral body,C3) of the cervical spine were measured by CT in the end-range cervical positions fixed by gimbal mounting: The data were analyzed to study the mechanism of action of vertebral artery space form out of neurohumoral regulation control.    Results    The end-range rotation and the end-range extension rotation caused homolateral vertebral artery stenosis (P<0.05), not on heterolateral (P>0.05), and narrow areas always appeared on upper part of C2 vertebral body 1/3.   Conclusion    (1)The composite vitro vertebral artery model loaded can be used to analyze the influence of different end-range cervical rotation on the vertebral artery lumen with CTA; (2) The end-range rotation and the end-range extension rotation caused homolateral vertebral artery stenosis out of neurohumoral regulation control, and narrow areas always appeared on upper part of C2 vertebral body 1/3 (The outside of the atlantoaxial lateral mass).

    Objective To compare and analyze the biomechanical stability of 3 different fixations for the posterolateral tibial plateau fracture. Methods The posterolateral shearing fracture models were created in 18 synthetic tibia(Synbone) and randomly assigned into 3 groups.The fractures were fixed by horizontal belt plate (Group A), lateral low-profile anatomical LCP (Group B) and posterior reconstructive plate(Group C). Vertical displacements of the posterolateral fragments and failure loads were measured under axial loads including static loading test (from 300 N to 1050 N), fatigue loading test and damage loading teat. Results  The vertical displacements in groups A,B,and C were respectively (2.77±1.79)mm, (2.69±1.14)mm, and (1.62±0.60)mm under fatigue loading test, the total load 1050N with 10 mm/min and repeat load 10000 times.There was no significant difference between the groups (P>0.05). After the damage loading test, the final force in groups A,B,and C were respectively (2055±263) N, (1968±209) N and (2272±130) N, and there was significant difference among the groups (P<0.05). Conclusion Horizontal belt plate fixation for isolated posterolateral tibial plateau fracture can bring sufficient stability for the posterolateral fragment.

    Objective To investigate the anatomical structure of the left lateral lobe, and to compare the differences between “the two-step separation approach” and the traditional method in laparoscopic left lateral sectionectomy(LLLS).  Methods  We retrospectively analyzed the clinical data and imaging information of 46 cases of laparoscopic left lateral lobectomy with "no fissure the falciform ligament of liver using two-step separation approach" from June 2014 to April 2017 in our hospital. Statistical analysis was performed for comparison data to patients with traditional laparoscopic left lobectomy before 2014. Results  There was no significant difference between the two groups in gender, age, course of disease, location and size of tumor, preoperative liver function, liver reserve function (P>0.05); intraoperative blood loss, postoperative hospitalization time of patients in the two groups were close, and there was no significant difference between the two groups (P>0.05) . The duration of operation in “the two-step separation approach” group was significantly shorter than that in the control group (P<0.05). Conclusion The modified “The two-step separation approach” in laparoscopic left lateral hepatectomy does not involve severance of falciform ligament, thus circumventing unnecessary operation, and reducing the operation time; The method is easy to learn and has become the gold standard for left lateral hepatectomy when taking into account the development of laparoscopic techniques.

    Objective To identify the surgical indication of one stage posterior approach for resection of cervicothoracic junction schwannoma. Methods From Jan 2005 to Jan 2016, cervicothoracic junction schwannoma cases admitted in our department were selected according to following indication: ①epidural tumor; ②lateral growth pattern which went through C6/7, C7/T1, T1/2 foramen; ③the diagnosis of schwannoma was suggested by MRI. ASIA motor score, radiological images features, operative time, bleeding amount, complications were recorded and analyzed after follow-up.  Results 14 cases were collected, including 8 male and 6 female, with an average age of 51.1y (28~74 y). 10 case were successfully managed by one stage posterior approach for resection of cervicothoracic junction schwannoma; however, 4 cases were sent to head and neck surgery department for second stage anterior approach tumor resection. The largest tumor size within the vertical line of the tip of transverse process was the indication of one stage posterior operation. All cases were successfully followed up for 10 to 48 month, averaging 23.9 month. There was no recurrence, and operative segments were solid fused. Preoperative ASIA motor score was improved from 81.71±6.63 to 97.14±1.29 at final follow-up, and the difference was significant(P<0.05). Conclusion MRI should be used to diagnose the tumor preoperatively. After careful patient selection, one stage posterior approach for resection of cervicothoracic junction schwannoma could be carried out with a satisfied clinical outcome, few complication and good prognosis.

    Objective  To analyze intrahepatic recurrence after regular hepatectomy and irregular hepatectomy in hepatocellular carcinoma patients, aiming at improving therapeutic effect.   Methods   116 hepatocellular carcinoma patients surgically treated in our hospital from November 2013 to June 2014 were enrolled as subjects. All patients were randomly divided into regular hepatectomy and irregular hepatectomy group, 58 cases in each group. The liver function after six months, the recurrence rate after one year and the survival rate in two groups were compared. Results The serum albumin (ALB) in regular hepatectomy group was significantly higher, while TBIL, glutamic-pyruvic transaminase (ALT) and total bilirubin (DBIL) were significantly lower than that in irregular hepatectomy group at six months after surgery. T test showed that the differences between the two groups were all statistically significant (P<0.01). The 1-year recurrence rate in regular hepatectomy group was significantly lower than that in irregular hepatectomy group. 1-year survival rate in regular hepatectomy group were significantly higher (P<0.05). The differences between the two groups were statistically significant (P<0.05). Conclusion Regular hepatectomy treatment to hepatocellular carcinoma patients has better liver functional recovery, lower intrahepatic recurrence rate and higher survival rate. It can significantly improve the quality of life of patients.

    Objective To investigate the potency of the platelet-rich plasma (PRP) combined with core decompression and allogeneic fibula rod support for the treatment of the osteonecrosis of the femoral head. Methods From July 2014 to May 2017, 26 cases (35 hips) of osteonecrosis of the femoral head were treated with platelet-rich plasma combined with core decompression and allogeneic fibula rod support (PRP group) and 24 cases (32 hips) with core decompression and allogeneic fibula rod support only (control group). At 1, 3,6 months after operation, Patients were followed up and evaluated using Harris hip score、VAS score and radiography. Results All the patients(67 hips) were followed up thoroughly and the follow-up periods were 6 months. At 6 months after operation, the shape of the femoral heads of the PRP groups was good with no deformation and collapse. The location of the top of the allograft fibula rod was perfect with no fibula rod out. The fibula rods made integration with the surrounding bone without infection. Harris score of PRP group before and after treatment increased from 67.82±9.61 to 88.45±6.02, Harris score of control group before and after treatment increased from 69.74±8.26 to 78.36±6.17;VAS score of PRP group before and after treatment increased from 4.6±1.2 to 1.7±1.0, VAS score of control group before and after treatment increased from 4.3±0.9 to 2.3±1.1. There were statistically significant differences in the pre-therapy and post-treatment groups (P<0.05) and there were statistically significant differences in the postoperation between the two groups (P<0.05). Conclusions Use of the platelet-rich plasma (PRP) in clinic can ease hip pain and improve joint function, and it is highly valuable in clinical use.

    Objective To evaluate the diagnostic value of ultrasonography guided fine needle aspiration for testis tissue.  Methods  The tissue slices of the testis through fine needle aspiration and core needle biopsy were collected from January 2015 to December 2016, the difference in the number of microtubules was compared. In addition, the differences in other ingredients (skin, blood vessels and fibrosoft tissue) and hematoma were analyzed. Results The testicle histopathological sections collected by core needle puncture or fine needle aspiration presented more than 20 seminiferous tubule sections for 8 patients in the core needle group and 11 patients in the fine needle group , without statistically significant difference between the two groups (χ2=2.087,P=0.149); additional components were present in the sections for 19 patients in the core needle group and 1 patient in the fine needle group, with statistically significant difference between the two groups(χ2 =24.203,P=0.000); 5 patients in the core needle group and 0 patient in the fine needle group experienced post-puncture hematoma, with statistically significant difference between the two groups(χ2=4.70,P=0.032). Conclusion The fine needle aspiration of testicular tissue is satisfactory, and almost no other components interfere with it, and have great clinical application value.

    Objective To investigate the efficacy of treatment of nonunion and delayed union with local injection of nerve growth factor. Methods 86 patients  with limb fracture nonunion treated in our hospital between January 2014 to January 2016 were in enrolled in this study which was randomized into the control group (43 cases) and observation group (43 cases). Patients in the control group underwent a modular external fixation combined with conventional rehabilitation therapy; patients in the observation group received additional local injection of nerve growth factor treatment. The therapeutic efficiency was compared in the two groups. The maximum diameter of callus within and over 4 weeks, the volume of callus, fracture healing time, quality of life and incidence of adverse reactions were also compared. Results Total effective rate was 76.7% and 95.3% in the observation group and the treatment group, respectively, with significant difference detected(χ2 = 8.39, P <0.01); the healing time of tibia, femur, humerus, ulna, clavicle, fibula and radius was significantly shorter in observation group than the control group (P<0.01); no significant difference in the maximum diameter of callus was found between the two groups (P>0.05); 4 weeks after treatment, the maximum diameter of callus in observation group was significantly higher (t=2.576, P<0.01); callus occurrence time, and fracture healing time was significantly earlier in the observation group than in the control group (P<0.01); no significant difference in the incidence of serious adverse events within 8 weeks were found in the two groups of patients (P>0.05). Conclusion Clinical efficacy of local injection of nerve growth factor in the treatment of nonunion and delayed union in patients with significant, which can significantly improve the treatment of fractures of efficiency, promoting callus formation and fracture healing, and lowering incidence of adverse reactions in patients, improving treatment safety, making this method worth popularizing in clinic.

    Objective To investigate the meaning and the surgical techniques of performing tonsillectomy by endoscopic surgery. Method 32 patients were treated by tonsillectomy (incising group) and 32 patients were treated by conventional stripping surgery (stripping group). The operation time, operation bleeding parameters, albuginea shedding as well as postoperative pain scale were compared and analyzed.  Result The incising group has much shorter operation time, milder postoperative pain and less blood loss (P<0.05).  However, albuginea shedding and the time returning to normal diet did not significantly differ (P>0.05).  Conclusion  The tonsillectomy by endoscopic surgery is safer, faster with less postoperative complications than conventional stripping surgery, which is worth popularizing.
