摘要

目的    回顾分析达芬奇机器人下胆道生理重建术在治疗胆道损伤后胆道狭窄方面的疗效。 方法    2014年10月年至2025年10月间我中心收治的胆道损伤后出现胆道狭窄的患者,其中17例接受了达芬奇机器人下胆道生理重建术,将此17例作为机器人手术组;另选取同时期同手术组相同病例开腹行胆道重建术17例作为对照组,对手术过程、术后并发症、术后恢复情况以及随访结果进行对比、分析。   结果    机器人手术组平均胆道重建时间长于开腹组(P<0.05),但总体手术时间、术中出血量、术后卧床时间、术后住院时间明显短于开腹手术组(P<0.05),两组病人术后胆漏发生率没有明显差异(P>0.05),两组病例3年内胆道再狭窄率均为0。   结论    达芬奇机器人下胆道生理重建术是一种安全、有效、可行的手术方式,能够缩短整体手术时间、较少腹壁创伤、加速术后康复。

Abstract

 Objective    To explore the efficacy of biliary physiological reconstruction with the Da Vinci Robot in treating biliary strictures following biliary trauma.    Methods    From October 2014 to October 2025, patients with biliary strictures following biliary tract injuries were admitted to our center. Among them, 17 cases underwent biliary physiological reconstruction using the da Vinci robotic system, were designated as the robotic surgery group. Another 17 cases of open surgery for biliary reconstruction in the same period and surgery group were selected as control group. The surgical procedures, postoperative complications, recovery status, and follow-up outcomes were compared and analyzed between two groups.   Results    The average biliary reconstruction time in the robotic surgery group was longer than that in the open surgery group (P<0.05), but the overall surgical time, intraoperative blood loss, postoperative bed rest time, and postoperative hospital stay were significantly shorter than those in the open surgery group (P<0.05). There was no significant difference in the incidence of postoperative bile leakage between two groups of patients (P>0.05). The restenosis rate of biliary tract within 3 years was 0 in both groups of cases.   Conclusions   Da Vinci robotic biliary physiological reconstruction surgery is a safe, effective, and feasible surgical method that can shorten surgical time, minimize abdominal wall trauma, and accelerate postoperative recovery.
