摘要

目的    探讨特制股骨近端静态托举装置在老年股骨粗隆间骨折闭合复位髓内钉手术中的应用价值。  方法    回顾2024年1月至2025年12月本院157例老年股骨粗隆间骨折患者,所有患者接受闭合复位髓内钉内固定术治疗,根据术中是否应用特制股骨近端静态托举装置分为研究组(79例,2025年1月~12月)和对照组(78例,2024年1月~12月),比较两组患者围手术期指标(术中出血量、术中撬拨率、手术时间、术后稳定性)、骨折复位情况(复位优良率、正性支撑率)、术后康复(疼痛、首次下床时间、髋关节功能)和并发症情况。  结果  研究组患者术中出血量、术中撬拨率、手术时间低/短于对照组,术后稳定性评分高于对照组(P<0.05)。研究组患者复位优良率、正性支撑率高于对照组(P<0.05)。研究组患者术后1 d VAS评分、首次下床时间低/短于对照组,术后1月Harris评分高于对照组(P<0.05)。两组术后并发症发生率无统计学差异(P>0.05)。  结论    老年股骨粗隆间骨折患者闭合复位髓内钉手术中应用特制股骨近端静态托举装置可以优化围手术期指标,促进骨折复位,提高患者术后早期康复状态。

Abstract

Objective    To explore the application value of a special proximal femoral static lifting device in closed reduction and intramedullary nail surgery for the elderly with femoral intertrochanteric fractures.   Methods  A retrospective analysis was performed on the case data of 157 elderly patients with femoral intertrochanteric fractures who underwent closed reduction and intramedullary nail fixation in the hospital between January 2024 and December 2025. According to intraoperative use of the special proximal femoral static lifting device, patients were divided into a control group (78 cases, January-December 2024) and a study group (79 cases, January-December 2025). The perioperative indexes (intraoperative blood loss, intraoperative prying rate, operation time, postoperative stability), fracture reduction (good reduction rate, positive support rate), postoperative rehabilitation (pain, the first leaving bed time, hip function) and complications were compared between the two groups.    Results    The intraoperative blood loss, intraoperative prying rate and operation time in the study group were lower than those in the control group, and score of postoperative stability was higher than that in control group (P<0.05). The good reduction rate and positive support rate in the study group were higher than those in the control group (P<0.05). VAS score at 1d after surgery and the first leaving bed time in the study group were lower than those in the control group, and Harris score at 1 month after surgery was higher than that in the control group (P<0.05). There was no significant difference in the incidence of postoperative complications between the two groups (P>0.05).   Conclusions   The application of a special proximal femoral static lifting device in closed reduction and intramedullary nail surgery for the elderly with femoral intertrochanteric fractures can optimize perioperative indexes and promote fracture reduction, which is beneficial to improve early postoperative rehabilitation status.
